About this position

At Cocoon House we empower young people, families, and communities in Snohomish County to break the cycle of homelessness through outreach, housing, and prevention. As a Coordinated Entry Navigator, you will be responsible for helping young people in Snohomish County access housing services, connecting them to the resources they need, and ultimately helping them transition into permanent housing. You will help coordinate with various community partners to ensure that those in need are connected to the best services available. The successful candidate will be a strong advocate for those in our community facing homelessness, connecting to community resources, and have a passion for improving their lives. Coordinated Entry Navigator Duties & Responsibilities

Full-Time| On-site in Everett & Snohomish County

Wages: $20-22/hr

  • Provide outreach to people experiencing homelessness and connect them to housing and homeless services
  • Conduct initial assessments to determine eligibility for Coordinated Entry services
  • Develop action plans and coordinate services with other providers
  • Collaborate and partner with other Cocoon House Programs, law enforcement, resource centers, schools, churches, and detention centers to identify young adults who need applicable services.
  • Provide case management and support services for individuals
  • Assist clients in applying for housing and other benefits
  • Identify and link individuals to available housing resources
  • Maintain client records and document all interactions
  • Update database with client information and progress
  • Help clients locate and secure stable housing
  • Follow up with clients to ensure housing stability and successful tenancy
  • Provide referrals to other services such as employment and education assistance, transportation, health care, and other community resources
  • Participate in regular meetings with other service providers to discuss client progress and coordinate services

Requirements

General:

  • High school diploma/GED or high school equivalency
  • Must be 21 years of age or older
  • 6 months minimum of experience working with youth
  • DCYF background clearance
  • Acceptable driver's abstract with no more than 2 violations within last 3 years
  • Possess a valid driver's license, auto insurance, and reliable transportation


Physical:

  • Ability to effectively communicate with all internal and external clients
  • Be able to hear voices and alarms.
  • Ability to read and comprehend a variety of materials
  • Ability to operate phone, computer, and record information for files and logs
  • Must be able to lift up to 50 Ibs
  • Must have ability to run up and down indoor and outdoor stairwells in case of youth emergency.
